Ok, so I have downloaded the game and unzipped it... how do I make it run? There is no install file, all I have is the main game .exe which won't run because it isn't installed.
Does it need to be installed? If not, does it need to be run in an earlier windows version emulator and not in win10?

They are not meant to be installed (at least most of the ones I tried). Just run the main .exe file and you are good to go.

I think that the problem is that this is an expansion pack, probably need to download Theatre of War 2 main game before

What the f? I had this in my library and wanted to download it now. Are you implying that some of the stuff in our libraries can evaporate from one day to the next? That's a terrible precedent.
